Meal Programs

Applications and forms for meal programs can now be completed online! See the options below and, if applicable, complete the appropriate form.

NOTE: To apply for a fee waiver, families must complete one of the forms below to determine eligibility.

What: FREE AND REDUCED-PRICE MEALS APPLICATION and Fee Waiver Eligibility

A program offering free or reduced-price meals (breakfast and lunch) in accordance with guidelines set forth by the National School Lunch and School Breakfast Programs.

COMMUNITY ELIGIBILITY PROVISION (CEP) FORM and Fee Waiver Eligibility

A provision of the federal lunch program that provides no-cost meals (breakfast and lunch) to all students at participating schools, helping expand access to  nutritious meals.

Who: Families wishing to apply for free or reduced-price meals with a student attending Bryant, Carver, Eisenhower, Hempstead, Hoover, Irving, Kennedy, Roosevelt, Sageville, Senior, Table Mound, or Washington. ALL families with a student attending Audubon, Fulton, Lincoln, Marshall, Prescott or Jefferson.
How: Complete the Free and Reduced-Price Meals Application

NOTE: If a member of your household:

  • receives federal benefits
    • food assistance
      (also known as SNAP)
    • FIP/TANF
  • receives a DHS letter
  • is homeless
  • is a foster child

you do not need to complete the Free and Reduced-Price Meals Application. To have your fees waived, contact your school office or the district food and nutrition office to receive a Fee Waiver Benefits Form.

Complete the Community Eligibility Provision (CEP) Form

NOTE: If you have students in both a CEP and non-CEP school, please complete both forms.
